Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management and utilization management. Full Time that will work 8-hour Day or Variable shifts.
Experience:
A RN with 20 years of RN experience will not be required to obtain a BSN.
Education:
Bachelor's Degree in Nursing or equivalent experience.
Master's Degree in Nursing or Healthcare related field.
Any previously agreed upon effective date for obtaining a Bachelor's degree in nursing between individual and facility will remain in effect.
Exceptions will be considered on a case-by-case basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader.
RNs employed prior to 1/1/17 are encouraged to obtain a Bachelor's degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment.
Certifications/Licensures:
Upon hire: California Registered Nurse License.
2 years of experience in healthcare related field (Acute, Ambulatory, Post-Acute, etc.).
2 years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager. TIP candidates must have experience in the same type of nursing unit in which the CM position is available.
Upon hire: CCM or ACM preferred.
Special Skills:
Must be able to use independent decision-making skills in a wide variety of situations.
Frequent contact with employees, physicians, customers, community individuals and volunteers.
Must be self-motivated to complete work in a timely manner to meet deadlines.
Experience with Core Measures, Quality Indicators, Case Management, and Utilization.
