OVERVIEW

Join our dynamic team as a Youth Case Manager and make a lasting impact in the lives of young people! In this rewarding role with Boys and Girls Club of Santa Monica, you'll have the unique opportunity to work closely with youth aged 10-18, guiding them to set and achieve personal goals that keep them on a positive path and away from violence. Your support will extend beyond the individual, as you'll also collaborate with family members and caretakers to create a strong network of support.

As a Case Manager, you'll be at the forefront of community outreach, building trust and meaningful relationships with both families and at-risk youth. You'll provide essential case management services, ensuring every step aligns with our program's vision and goals, along with grant requirements. From maintaining detailed records to compiling impactful reports, your organizational skills will be key in driving our mission and impact forward.

JOB RESPONSIBILITIES:

•Attends site and/or in-service meetings for the purpose of conveying and/or gathering information required to perform functions

•Assists with participant outreach by preparing and distributing outreach material, making presentations, phone calls, and scheduling meetings

•Maintains a variety of manual and electronic documents/reports, files, and records for the purpose of providing up-to-date information and historical reference in accordance with established administrative guidelines and legal requirements. This may include participant information.

•Assists with assessing participants' needs, situations, personal strengths, and support networks to work with youth and determine appropriate goals

•Coordinates with appropriate organizations and services including, but not limited to, social services, physical and mental health services, housing support, employment education and substance use disorder treatment

•Monitors and tracks milestones such as, employment acquisition, and/or educational progress

•Responds to inquiries from a variety of internal and external parties (e.g. schools, partners, government agencies, general public, students, etc.) for the purpose of providing information, facilitating communication among parties and/or providing direction

•Meets with youth and families in compliance with program requirements, researches and refers participants to community resources

•Tracks youth participation in positive activities and provides supports to engage youth in these activities

•Responsible for all tasks related to holding case management meetings with youth and families

•Illustrate a commitment to equity, inclusion, and accessibility.

Requirements

EXPERIENCES

•Providing case management, mentoring, and supportive services to at-risk youth or populations

•Demonstrated experience navigating complex government, social service, academic, or health institutions.

•Demonstrated experience serving diverse populations and leading with a trauma-informed lens.

•Working in school-based systems, youth serving nonprofits, community organizations, advocacy, and or/ support and community groups.

•Using data management systems, HIPPA compliance, case management tools

•Training or background in motivational interviewing.

•Hold a valid drivers' license and pass motor vehicle driving record check.

•Pass criminal background check.

•Pass (negative result) a TB test.

•CPR and First Aid Certifications preferred.

EDUCATION LICENSURE and CERTIFICATION

•Some colleges are in social work, psychology, sociology, human services.

•Bachelor's degree in social work, psychology, sociology, human services.

•Two years of increasingly responsible experience working with youth populations, particularly those in areas with high rates of violence.

•Bilingual skills preferred.

QUALIFICATIONS

Knowledge of:

•Working with youth in a trauma-informed environment (training available to strengthen these skills).

•Assessing youth wellness, needs, and abilities and using the knowledge of them to develop plans.

•Listening to youth/clients' concerns and providing coaching.

•Recording youth/clients' progress including charting referrals, home visit, and other notable interactions.

•Organization and coordination of specialized and responsible clerical functions including project and time management; case documentation, appropriate use of technology for communication.

•Strong written and verbal communication.

Working conditions are primarily an office setting, long hours of close work at desk and / or computer terminal; frequent interruptions via in-person contact, phone calls, and email. Occasional travel will be required to various communities and businesses in the region. Physical exertion may be required to lift supplies and equipment. The ability to travel commercially and to communicate orally and in writing / typing are required to perform the essential functions of the job.

Work is performed mainly during regular office hours; however, incumbents may be required to attend meetings or agency events outside regular office hours.
