Position Summary

The Case Manager III - Community Intervention will play a vital role in providing comprehensive case management services to individuals and families in the community who are at risk of or have been affected by violence. This role involves assessing needs, developing intervention plans, coordinating services, and advocating for clients to ensure they receive appropriate support and resources.

The Case Manager III will work closely with community partners and stakeholders to implement violence prevention and intervention strategies. They will conduct thorough assessments, provide crisis intervention, and develop safety plans to address immediate and long-term needs. Additionally, the Case Manager III will collaborate with internal and external teams, facilitate support groups, and track progress towards goals.

Competitive pay rates: Bachelor's Degree $22.00-$29.00; Masters Degree $32.21 -$34.00

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:
Conduct comprehensive assessments to identify client needs, strengths, and barriers Develop and implement individualized intervention plans Coordinate and monitor services to ensure clients receive appropriate support Provide crisis intervention and de-escalation support Advocate for clients and collaborate with community partners to access necessary resources Facilitate support groups and educational workshops Track and document client progress and outcomes Collaborate with internal teams and attend meetings and trainings as required Stay updated on best practices and emerging trends in the field of community intervention Requirements Minimum Qualifications (Knowledge, Skills, and Abilities) Minimum of 5 years of experience in case management or a similar role Knowledge of trauma-informed care and experience working with individuals impacted by violence Excellent assessment and problem-solving skills Strong communication and interpersonal skills Ability to work effectively with diverse populations Proficiency in case management software and computer applications Valid driver's license and access to reliable transportation Ability to maintain confidentiality and adhere to ethical guidelines Professional certification or licensure in social work (LCSW) is preferred Benefits
