Job Title: MSW Social Worker
Job Location: Summit, NJ
Job Duration: 1 Month+ (Possible Extension)
Shift: 8 am – 4 pm or 8:30 am - 4:30 pm
Job Summary: · Collaborate with a multidisciplinary team to deliver high-quality, patient-centered care.
· Address social determinants of health to support safe care transitions and improve outcomes across the facility.
Job Responsibilities: · Patient Assessment, Referrals, and Intervention (60%): Conduct psychosocial assessments, address barriers to care, and support health and social needs.
· Documentation (20%): Maintain accurate and timely records of assessments, interventions, and referrals.
· Meetings and Program Development (10%): Participate in meetings and contribute to program development.
· Collaboration with Agencies (10%): Coordinate with internal and external agencies for comprehensive patient support.
Job Functions: · Support High-Risk Patients: Address care barriers, improve health outcomes, and facilitate resource connections.
· Provide Psychosocial Support: Assist with social, economic, and behavioral health needs, and guide patients in achieving health goals.
· Maintain Best Practices: Ensure culturally-sensitive care, equitable access, and reduce health disparities.
· Conduct Assessments and Develop Plans: Perform screenings, create care plans, and offer telephonic/virtual support.
· Ensure Effective Collaboration: Communicate with the Care Coordination team and community agencies for comprehensive care.
· Participate in Meetings: Engage in interdisciplinary forums, training, and provide consultative support for community health workers.
· Educational Requirements: Attend at least three relevant educational programs annually.
Additional Tasks: Perform other duties as assigned by manager or leadership.
Education: · Master's degree in social work from an accredited school of social work required
Experience: · 3-5 years in health-related field, hospital experience preferred
· In Patient Exp. Required
· Behavioural Health background preferred
