Position: Adult Day Services Day Habilitation Without Walls (DHWW) Program Coordinator Summary: Block Institute is seeking a dedicated and experienced Program Coordinator to oversee and manage the day-to-day operations of our Day Habilitation Without Walls (DHWW) program. This program provides community-based services to participants, and the Coordinator will be responsible for coordinating activities, ensuring compliance with regulatory requirements, and supporting participants in achieving their individual person-centered goals.

Essential Job Duties and Responsibilities: Coordinate and schedule community-based activities, outings, and volunteer opportunities. Foster community partnerships to enhance our service delivery. Ensure implementation of community-based activities and service delivery. Provide direct support and supervision during community-based activities. Monitor program effectiveness and make necessary adjustments to improve service delivery. Foster a supportive and inclusive environment that promotes independence and community integration. Conduct a walkthrough of the physical plant. Address and resolve any issues or concerns raised by participants or their families. Recruit, train, and supervise program staff and volunteers. Conduct regular staff meetings, provide ongoing training, and support professional development. Evaluate staff performance and provide constructive feedback. Ensure staff adherence to organizational policies and procedures. Develop partnerships with community organizations, businesses, and other service providers. Advocate for the inclusion and participation of the participants in community activities. Represent the program at community events, meetings, and other relevant forums. Provide feedback and coaching for program implementation and group configuration meetings for individuals and staffing. Manage program supply inventory listing and programming schematic configuration (onsite, community, etc.) Assist in developing and managing the program budget. Monitor expenses and ensure efficient use of resources. Use discretion and independent judgment to compare and evaluate possible courses of conduct, impacting business operations and financial outcomes. Has the authority to waive or deviate from established rules or policies, maintaining communication with direct supervisors and upper administration. Conduct ongoing training, supervision, and oversight for all supervisory, direct support level, and clinical team members. Attend and facilitate scheduled life plan meetings and interdisciplinary team meetings regularly. Provide initial and ongoing training to all supervisory and direct support level staff. Conduct routine program reviews to ensure compliance with regulatory and billing standards. Provide supervisory coverage in the absence of a Program Supervisor. Maintain certification in SCIP, CPR, First Aid/OSHA, Code Red/Green, and all other mandated training. Ensure all pertinent information is promptly shared with the Director of Adult Day Services. Oversee all monitoring systems and assess and develop new systems and procedures. Provide appropriate coverage/supervision via phone and email to assigned supervisory-level staff during programming hours. Ensure that all Adult Day Service DHWW programs are maintained cleanly and safely. Mandatory attendance at required in-service training and compliance with all regulations and Block Institute policy and procedure requirements. Consistently at work and on time, ensuring responsibilities are covered when absent. Supervisory Responsibilities: Directly supervises all Program Supervisors, Group Leaders, and Assistant Group Leaders as assigned to Adult Day Services program sites.
