General Description

Our Advanced Practice Registered Nurses (APRN) serve our community through providing direct clinical care of clients, including ordering of lab tests, counseling of patients, developing treatment plans and prescribing medication (within the scope of their education and certification of the collaborative agreement with their collaborating physician). Essential Functions




Works with staff, as a team, to provide high quality care to our patients by identifying and assessing health concerns, examines the patient based on the health concern, and creating a plan specific to the patient needs.




Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.




Provides clinical consultation to other staff members and supervision of staff and students as assigned.




Demonstrates adherence to National Patient Safety Goals and plan.




Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational trainings offered on a regular basis for professional development.




Scheduled Hours: Based on Clinic Needs


40 Hours per Week


Monday - Friday with a weekly evening rotation and a monthly Saturday rotation




Knowledge Skills Abilities and Education Required




Master's Degree in Nursing or postmaster certification as an APN required.


Appropriate certification by National certifying body as required by state law (ANCC, NCCC, NAPNAP, AMCB, or ACC) is required.


Current Illinois licensure as a Registered Nurse or Advanced Practice License is required.


Minimum of 2 years of APRN experience in Primary Care




Supplemental Information




As a public health department, we believe in the science, data, and power of vaccines, like the influenza vaccine, to protect against and prevent illness. Therefore, we require all Health Department staff to receive the annual influenza vaccine unless they have an approved medical or religious exemption.




Any offer of employment is conditional upon the successful completion of a background screening, drug and alcohol testing and may include a pre-employment medical exam.

Pay: $98,453.00 - $124,656.00 per year
