DESCRIPTION OF POSITION The Care Coordinator is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. Collaborative Care is a unique model for delivering mental health care in the primary care setting. Care Coordinators support and coordinate the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.QUALIFICATIONS Professional/Educational Graduate of an accredited college or university with a Master's Degree in Social Work or related field required.3-5 years' experience in a clinical or community health setting.Previous Care Coordination and/or Case Management experience preferred.Previous experience with health IT systems and data reports preferred.Previous experience with mobilizing community resources, navigating through the healthcare continuum and working with disparate populations preferred.Ability to speak a relevant second language preferred.Working knowledge of differential diagnosis of common mental and/or substance abuse disorders, when appropriate.Experience with assessment and treatment planning for common mental health and/or substance use disordersState of IL license as a LCSW or LCPC.DCFS Mandated Reporter Certificate of Completion required.Personal Demonstrates evidence of essential leadership, communication, education and counseling skills.Proficient in communication technologies (e-mail, cell phone, etc).Effective organizational skills, demonstrates ability to maintain accurate notes and records.Ability to maintain effective and professional relationships with patient and other members of the care team.Demonstrated ability to collaborate and communicate effectively in a team setting.Demonstrates a positive attitude and respectful, professional customer service.
