*White Glove Community Care is* one of NY's largest home care agencies and is seeking to hire an energetic and responsible *Home Care Coordinator*. Our network of highly qualified Home Health Aides offers dedication and compassion to the people we serve. We take pride in delivering only the highest quality care for our patients. We are currently looking for a *Home Care Coordinator* to work in a full-time position.




*Responsibilities:*




*You will receive excellent in-house training which will help you develop a thorough understanding of your role*.




*Home Care Coordinators *have a wide range of responsibilities, which can include:


* Effectively communicate with patients, family members, contracts, clinicians, HHA's and/or other departments depending on issues or changes


* Complete all required tasks based on agency guidelines and policies


* Schedule work assignments with regards to Home Care Worker training and experience.


* Minimize volume of open cases


* Document all communication in HHA Exchange.


* Consult with family members and other support, to ensure that care is on-going and that all client needs are identified and met.


* Advise Home care worker of discharges, changes in service days/ hours or any other as determined.


* Complete required daily, weekly, monthly reports based on implementation


* Manage patients' overtime and travel time requests.


* Use Smart Map and other tools to find home care workers and provide directions and availability.


* Complete incident reports.


* Manage mutual and linked cases.


* Open emails and reply in a timely manner and listen to voicemails and reply in a timely manner


* Open faxes and act in a timely manner


* Adapts easily to any other tasks assigned by manager


* *HHAEXCHANGE knowledge preferred*


* *Bilingual (English, Spanish)*




*Qualifications:*


* Minimum High School Diploma


* Prior experience a plus, but willing to train the right candidate.


* Excellent customer service skills


* Strong computer and writing skills, including knowledge of Microsoft Outlook


* Knowledge of HHA exchange \*\*is a plus!\*\*\*

Job Type: Full-time




Pay: $25.00 - $28.00 per hour




Expected hours: 35.5 per week
