Responsibilities:Works with the interdisciplinary team to execute a customized plan of care for each unique patient.Guides the patient through the treatment process, reviewing instructions, and scheduling appointments.Facilitates interaction between patients and their physician and referring physician(s) as appropriate. Navigates the patient and family across care settings according to the established patient care plan and clinical pathwaysUtilizes effective communication to ensure patient and caregiver(s) have appropriate information, understand, and have the capacity and tools to adhere to the individualized treatment plan.Contributes to quality-driven performance improvement initiatives.Requirements:Bachelor's Degree NursingCurrent New York State Registered Professional Nursing LicenseExcellent communication (both oral and written), interpersonal, and organizational skills.Demonstrate consistent professional conduct and meticulous attention to detail.Proficiency in Electronic Health Record documentation Minimum of two years experience in an outpatient medical office setting. Healthcare or Customer Service ExperiencePatient Navigation Certification (NYCBS will fully support our team member(s) in achieving and maintaining specialized certification.)Hours: Monday-FridaySalary up to $55/hr. based on experience.
