Description
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Care Specialist will be responsible for building rapport with patients and for organizing schedules for our Caregivers to ensure our patients are cared for. The Care Specialist will also be responsible for providing support in the monitoring of Homecare workers and ensuring that the program maintains compliance with DOH standards in conjunction with the HR Department. The Specialist is responsible for the following:
Producing daily reports including but not limited to the SANTRAX/SAM Exception daily report; addressing concerns raised with Home Health Aides, i.e. follow up calls, merging calls, etc. Reviewing open scheduling orders in the available system and Scheduling home health aides to permanent cases/shifts Ensuring client services are scheduled as prescribed on authorization documentation Understanding patient-specific care needs to identify appropriate caregivers to assign to cases Conducting reminder calls to ensure employees attend scheduled in-service and physical appointments Serving as On-Call Coordinator and back-up for other Care Coordinators as needed Communicating trends, patterns and concerns to the appropriate teams, including Coordination, Social Work, HR, Payroll, Administration, Risk Management, etc. Responsible for the comprehensive and timely documentation of all interactions in the required databases utilized by RiseBoro Homecare. Participating in various quality improvement activities, including contacting clients for feedback related to the quality of services Responsible for conducting annual homecare worker evaluations Reporting any homecare workers compliance issues as related to evaluation, in-service, annual physical and other requirements to Care Coordination Supervisor Serving as On-Call Coordinator and back-up for other Care Coordinators as needed Attending agency meetings, i.e. case conferences, supervision, etc. as well as staff development trainings Other related duties as assigned. COMPETENCIES 2-5 years of experience in homecare services or related field Knowledge of HCPLUS, Human Resources Administration (HRA) Long-term Web application, SANDATA/SAM, HHA Exchange or similar home care platforms Bilingual (English and Spanish) preferred Problem solving abilities while multi-tasking; ability to exercise sound judgment Excellent verbal and written communication skills Excellent customer service and organizational skills Knowledge of home care concepts, practices and procedures within a LHCSA Ability to maintain confidentiality at all times Commitment to organizations mission, vision and values Moderate to strong computer proficiency using standard office software programs required, in particular Microsoft Office; ability to utilize technology in daily work; experience with web-based applications
EDUCATION EXPERIENCE
HS Diploma or equivalent required; Bachelors preferred
