Job Summary
The Case Manager plays a vital role in supporting individuals by providing comprehensive case management services. This position involves assessing client needs and coordinating resources to ensure clients receive the expected outcome. The ideal candidate will possess a strong background in team work with expertise in cases follow up.
Responsibilities
Conduct thorough assessments of clients to identify needs and develop individualized action plan.
Coordinate services and resources for clients.
Monitor client cases progress and adjust plans as necessary to ensure optimal outcomes.
Provide crisis intervention and management support to clients facing urgent situations.
Maintain accurate documentation of client interactions, progress notes, and service plans.
Skills
Strong knowledge of Windows and Excel.
Bilingual
Ability to work collaboratively within a team environment.
Strong organizational skills with the ability to manage multiple cases simultaneously.
