EAC Network, a leading not-for-profit social service agency that supports over 62,888 individuals through more than 100 programs across Long Island and NYC, is looking for a dedicated Case Manager to join our Brooklyn Community Re-Entry Assistance Network (CRAN) program.
Position:Full-Time Case Manager - Community Transitional Case Management (CTCM)Location:Brooklyn, NYSchedule:Monday to Friday, 9:00 AM - 5:00 PM (Some travel to Rikers Island required)Compensation:$28.93 per hourReports To:Program Director
Why This Role is Vital:
As a Case Manager (CTCM), you will play a crucial role in guiding and supporting individuals with substance use and mental health challenges as they reintegrate into society. Your work will ensure that these individuals receive the care and resources they need to thrive.
Key Responsibilities:
Collaborate and Screen:Work closely with discharge planners on Rikers Island and the Court System to conduct screenings (in-person, video, and phone) for incarcerated clients before their release.
Facilitate Referrals:Use comprehensive screening results to coordinate referrals to mental health, drug abuse, or alcoholism facilities. Assist with transportation arrangements when needed.
Benefits Assistance:Help clients apply for and access public and private benefits, and connect them with community-based providers.
Case Documentation:Maintain detailed client records and progress notes, ensuring documentation is up-to-date and available for supervisory review.
Manage Caseload:Oversee a caseload of clients, monitoring their progress through regular case management appointments, progress reports, and collaboration with treatment teams.
Reporting:Prepare and deliver written and oral status reports to the criminal justice system regarding client progress.
Supportive Services:Research and recommend additional supportive services for clients.
Drug Testing:Conduct drug testing as required and make timely visits to treatment and correctional facilities.
What We're Looking For:
Education:Bachelor's degree in Forensic Psychology, Psychology, Criminal Justice, or a related behavioral science field.
Experience:Strong communication skills, both oral and written, and the ability to collaborate effectively with treatment and criminal justice systems.
Technical Skills:Proficiency in computer applications, including spreadsheets, Microsoft Office, calendar, and email. Familiarity with automated systems and database software is essential.
Additional Requirements:Ability to travel within Brooklyn and visit Rikers Island; proof of COVID-19 vaccination required.
