ESSENTIAL FUNCTIONS
The case manager will assist each client of the ACCESO program in meeting their needs with an emphasis on problem solving. He/she will support the coordination of clinical and social care for each client of the ACCESO program. Responsibilities will require continuous contact and communication with the clients of the program, clinical staff, administrative staff and community resources according to the needs of the people participating in the program.
REQUIREMNTS AND SKILLS
Education:
Bachelor's degree in social work.
Active Member of the College of Social Work Professionals of Puerto Rico
Permanent social work license
Experience working in direct service to mental health clients.
Knowledge of current laws relevant to the field of mental health, knowledge of the HIPPA-A Law, fraud and abuse, protection of minors in Puerto Rico and the United States.
Knowledge of DSM-V, ICD-11, billing to private health plans, Medicaid, Medicare
Skills in writing narrative reports
Oriented to teamwork, discussion, and follow-up of clinical and discharge treatment plans.
Excellent personal relationship skills
Ability to set priorities.
Knowledge of Programs: Microsoft Word, Power Point, Excel and others.
