Do you desire to deliver care in an empathetic, compassionate way - the way you'd want your own care to be?Essential Job Duties and Responsibilities: Responsible for performing, coordinating, and directing nursing care and services in a chronic-care facility for the end-stage renal disease patient in accordance with DaVita healthcare policies, procedures, and guidelines, OSHA, CMS, federal, state, and local regulations.Extend dialysis knowledge to others through training and coaching and assist in leading clinical initiatives.Supervise and monitor direct patient care administered by PCTs, LPN/LVNs, and RNs to ensure compliance with DaVita's policies and appropriate documentation of all delivered care. Responsible for the performance and duties of clinical teammates who are being directed or assigned.Perform direct patient care, including initiation and termination of dialysis, physical assessment of patients, taking and transcription of physician's orders, and drawing and documentation of ordered laboratory work.Analyze patient assessments, and laboratory data, and bring concerns to the attention of the physician. Participate in the development and implementation of the patient's short and long-term plan of care and monthly progress notes.Instruct patients on pre-and post-dialysis care and provide appropriate information on renal disease, prescribed dialysis treatment, and medical regimen. Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient's state of health and wellbeing.Act as a resource for colleagues with less experience as well as demonstrates the ability to transfer skill set to other clinics and work at other clinics if required.Create and change teammate and patient schedules as necessary.Other duties as assigned.Some details about this position:At least 6 months dialysis experience is required.Four day work weekCoordinate ESRD patient care and services (trend lab values, track metrics on vital signs, weight, outcome management, etc.)•Ensure you and your clinical team are delivering the highest level of care to our patients by ensuring their safety, comfort, and well-being.Training may take place in a facility or a training clinic other than your assigned home clinicPotential to float to various clinics during and after your trainingYou must have a flexible schedule and be able to work mornings, evenings, weekends, and holidaysWhat you can expect:Build meaningful and long term relationships with patients and their family in an intimate outpatient setting.Be a part of a team that supports and relies on each other in a positive environment.
