Salary: $$17.60 - $18.25 (higher salary with Medication Administration Certification) Job Type: Full Time Requirements of Position: CNA hiring skill level to support individuals with more specific medical needs in daily living tasks. GNA hiring skill level for elderly individuals in daily living tasks include: 1. Consistent attendance and punctuality. 2. Following instructions and responding to supervisor's direction. 3. Demonstrate sound time management and dependability. 4. Contribute to building positive Teamwork. 5. Demonstrate accuracy and thoroughness. 6. Exhibit effective communication skills, both written and oral. 7. Exhibit initiative and resourcefulness. 8. Adaptable to change. 9. Consistent demonstration of professionalism. 10. Ability to perform the physical demands of the position, including 50- pound lifting requirement. 11. Exhibit a friendly, compassionate bedside manner. 12. Computer knowledge and ability to document care notes and services provided in database. Duties of Position: Individual Program Planning 1. Participate in preparation of the PCP by completing initial annual assessments, and present recommendations for development of goals to the Community Living Coordinator. 2. Attend all PCP meetings, pre-PCP meetings, six month reviews, and special staffing's, as required; if not present, read and sign off as reviewed. 3. Work in cooperation with the Community Living Coordinator in development, revision, and implementation of each individual's PCP and all PCP recommended activities identified as the community living program services' responsibility. 4. Responsible for goals and daily record keeping of data, as designated in each individual's PCP. 5. Meet with the therapists/consultants and the community living coordinator to discuss issues and concerns as needed. 6. Assist the Community Living Coordinator in program development, using creativity to promote active engagement, independence, and involvement in the community. 7. Submit all paperwork in a timely manner- including van documentation, work orders, attendance, care notes etc. 8. Maintain knowledge of medical issues and conditions. Individual Related: 1. Supervise individuals and activities, ensuring active engagement throughout the day. 2. Report any concerns to the Community Living Coordinator (injury, illness, behavioral) and document in ICM within 24 hours. 3. Maintain the individuals' attendance alerting the Community Living Coordinator of any concerns. 4. Maintain open communication with families, caregivers, vocational providers, and professionals. 5. Provide additional support, when needed, to maintain integration in the community and recreational activities. 6. Administer medications to individuals as required. 7. Operate a safe work environment, including assurance of each individual's safety when loading and unloading vans. 8. Assist with daily transportation needs, as required. 9. Read the individuals' daily logs and report any concerns to the Community Living Coordinator; record in the log, as required. 10. CNA/GNA level of support to individuals. Inclusive of bathing, toileting lifting transferring, etc. 11. CNA/GNA level of support needed to support grooming, dressing, teeth brushing, shaving, nail care etc. 12. Plan and coordinate supervisor approval for appropriate leisure and social activities. 13. Promote and coordinate involvement with individuals' family and friends. 14. Encourage individuals to engage in self- advocacy. 15. Shop for and prepare nutritious meals according to established menus and/or nutritional recommendations. 16. CNA/GNA level of support to aid with feeding, following the Nursing Care Plan and feeding protocols. 17. Responsible for individual's funds and being accountable for receipts for all purchases. 18. Responsible for always meeting each individual's supervision requirements. Policy and Procedures: 1. Comply with the DHMH and Spring Dell Center's Policies and Procedures. 2. Support the agency's mission, goals, and values. Licensing: 1. Provide services that meet applicable licensing, certification, and accreditation requirements. 2. Demonstrate knowledge and understanding of the DDA Regulations. General: 1. Maintain accountability to the Community Living Coordinator. 2. Acquire and maintain all required training certifications including CNA/GNA license, Medication Administration, CPR/First Aid, DDA required and Van Driving certification. 3. Attend all additional meetings, training, and in-services as directed by supervisor, per regulations, and staff interest; adjusting schedule, as required. 4. Provide input to the Community Living Services goal planning, as part of the SDC strategic planning process. 5. Maintain a professional attitude with individuals, coworkers, caregivers, and other professionals. 6. Maintain a clean, safe home environment. Ensure the exterior/interior of the house is maintained in all appealing safe and sanitary manner. Ensure that all appliances, vehicles, and fixtures are maintained in operating condition. 7. Perform other duties and responsibilities as required within the scope of the position. 8. Maintain house account, petty cash, and checkbook. Must submit receipts for all purchases and balance account accordingly. Qualifications and Skills Minimum Requirements: Valid drivers license. At least 2 years driving experience and a clean driving record. Ability to lift 50 pounds. Administer medications. Basic computer skills. Experience working with disabled adults preferred.
