Director Of Medical Respite

JOB SUMMARY:

The Medical Respite Program provides people experiencing homelessness who are hospitalized with a safe, stable place to recuperate after discharge. The program provides short-term residential and rehabilitative medical care for patients. The program supports individuals who are too ill to recover from a physical illness or injury while living in a shelter or on the street, but who are not sick enough to require hospitalization.

Under the direction of the Chief Program Officer, The Program Director is responsible for the day-to-day oversight, management and achievement of the Medical Respite Programs, Recuperation in a Supportive Environment (RISE) Center and Sojourner House.

The position will work with our health partners to ensure that the program follows general acceptable standards of care to ensure that clients are receiving the appropriate level of care. This position is also responsible for ensuring that all services are delivered within the standards set by the agency and for program reporting. The Director will, in conjunction with the Chief Program Officer, design and implement programmatic enhancements in alignment with agency strategic objectives for program growth and development.

JOB RESPONSIBILITIES:

Services, Case Planning, and Management:

In conjunction with the Medical Director review requests for admission and make appropriate decisions based on available rooms and other factors such as vulnerability and availability of other shelter options.

Ensure that intakes and services plans are completed within a reasonable time after admission based on the client's health status.

Work with case managers to ensure that clients are following case plans and on track to achieve their personal goals within the timeframe stipulated in the case plan.

Coordinate with medical staff the transfer of medically stable participants to the next appropriate level of care/housing.

Maintain accurate and confidential client files in accordance with Housing Forward policies and regulatory bodies.

Ensure program staff are working with appropriate staff from Interim Housing and external partners to facilitate disposition of cases.

Approve all client's financial assistance requests according to funding and program policies and submit applicable documentation to the Finance Department for processing.

Ensure clients' requests and provision of concrete goods are processed in accordance with program policies and procedures.

Promote effective case management by ensuring Outreach staff are accessing appropriate community resources for participants, including but not limited to entitlement benefits (Food Stamps, TANF, SSI/SSDI, Medicaid, etc.)

Ensure proper and timely documentation of service provision and client progress.

Provides crisis intervention, as necessary.

CE Responsibilities:

Work with CE Dedicated Assessor to ensure key metrics are complete and correct in HMIS so that clients can be assessed for housing need on a timely basis and can be considered for all resources for which they are potentially eligible. Supervise staff responsible for making necessary changes in HMIS to ensure timely and accurate updates.

Coordinate with CE Dedicated Assessor and monitor program staff working on client documentation required for various housing processes to ensure timely and accurate completion.

Work with System Performance Team and CE Dedicated Assessor to understand, utilize and update agency By Name List (BNL) referrals and client progress trackers in SharePoint.

Staff Supervision:

Hire and train staff. Conduct annual performance evaluations.

Provide ongoing supervision, direction, coaching and support to staff including case consultation, interventions and diversion techniques with clients, crisis management and oversight of incident reports.

Ensure that staff complete the documentation of their work in a timely manner.

Coordinates the schedules of program staff, interns, and volunteers.

Ensure that staff completes the minimum required training set by the agency. Identify other training needs for staff and develop an individual plan to meet those needs.

Creates an atmosphere in which timely and high-quality information flows smoothly between self and others.

Encourages open expression of ideas and opinions; delegates and empowers staff; removes obstacles and provides needed resources.

Orient new staff, interns, and respective volunteers to program policies and procedures.

Administrative and Operations:

Regularly review Data Quality Reports in HMIS (Homeless Management Information System) to ensure the accuracy of data and data entry.

Work with Data & Analytics Manager to gather monthly statistics from HMIS and data for performance measures.

Assists with the reporting of government and private grants.

Oversee and participate in program audits and site visits.

Participates in quality improvement review of client files for completeness (e.g., self-sufficiency plans, case notes, releases of information including but not limited to HMIS consent forms, and exit summaries, etc.) and the archiving inactive/closed files.

Approves staff and time-off requests, and expense reports.

Attend collaborative meetings and community education events as requested.

Work with Chief Program Officer to maintain program manuals including updates to policies and procedures.

Review and discuss grievance reports with clients and staff and submit form to Chief Program Officer.

Program Planning, Development, and Evaluation:

Prepares and monitors program budgets.

Recommends service and program changes based on feedback from client satisfaction surveys. Work with Chief Program Officer to implement program policies and procedures to respond to client feedback.

Works with the Chief Program Officer in the ongoing development, implementation, and evaluation of the program.

External and Partner Relationships:

Develop and maintain relationships with partners and other community agencies that may provide additional services, on or off site to clients.

Participate in health and housing initiatives that involves Housing Forward.

Participate in community education and outreach as requested.

General:

Works as part of the team to provide support and assistance whenever needed.

Follows agency policies and procedures for the obtainment of information and the provision of services to clients.

Participates in and contributes to regular management meetings, staff training, client activities, community meetings, and other agency events.

Performs other duties as assigned.

Meet regularly with Chief Program Officer for supervision.

Actively participate in all-staff and leadership meetings.

Attend professional staff trainings as scheduled.

Attend networking meetings as assigned.

Represent the agency in a professional and ethical manner when meeting with area agencies, funding bodies, donors, and other professionals.

Maintain a team approach with all staff in all work-related situations and with the staff of other agencies/organizations when collaborating on shared programs/grants.

Other duties as assigned.

QUALIFICATIONS:

MSW or related master's degree with minimum two years supervisory experience.

A bachelor's degree with a minimum of three years' experience including lived experience in lieu of a master will be considered.

Demonstrates ability to provide supervision and coaching to a team of staff with different responsibilities.

Demonstrates organizational skills.

KEY COMPETENCIES:

Understanding of and sensitivity to the needs of individuals experiencing homelessness.

Basic familiarity with health care processes and policies including patient confidentiality.

Experience in leadership roles including supervision of staff in a social service setting.

Working with staffing agencies and contractors would be a plus.

Excellent written and verbal communication skills.

Crisis intervention, mental health and substance abuse recognition, employment readiness and accessing supportive services.

Consistent, reliable, and team oriented.

Knowledge of and understanding of community resources and services.

Good collaboration skills and the ability to work with people of diverse backgrounds.

Computer skills, including MS office, knowledge of or ability to quickly learn program databases.

Bi-lingual a plus.

Must be able to manage multiple responsibilities.

Must have a car, valid driver's license, and proof of insurance.
