Family Nurse Practicioner- Outpatient Fqhc Brooklyn Ny

A health services organization in New York City is actively seeking a licensed Family Nurse Practitioner (FNP) to join one of their federally qualified health centers (FQHCs) in Brooklyn. In this role, the Family Nurse Practitioner (FNP) will be responsible for providing high quality medical care to Adult & Pediatric patients, including preventive activities such as screening examinations, comprehensive health assessments, diagnosis and treatment of minor, acute and chronic medical conditions.

Responsibilities:
The Family Nurse Practitioner (FNP) will:
Formulate diagnosis and institutes treatment based on objective data gathered Make referrals, as indicated (i.e., specialty clinics) Participate in regular chart reviews and other activities Promote and maintain high quality medical care for the patients of the satellite health center Coordinate patient care with other clinicians Perform all procedures and duties applicable to private practice in an office setting Participate in telephone on-call schedule, as needed Perform other duties, as needed Qualifications: 2+ years of Clinical experience as a Family Nurse Practitioner (FNP) Successful completion of an accredited Nurse Practitioner program NYS Family Nurse Practitioner (FNP) license Desired Skills: Multilingual Community Health Center setting
