Western Wisconsin Health is seeking a dedicated Advance Practice Provider to join our new clinic in Spring Valley! We are looking for a provider who shares WWH's vision of creating healthier tomorrows for our patients and is passionate about supporting our community members along their wellness journeys.
Our ideal candidate will have experience providing continuing and comprehensive care for families and individuals of all ages. Responsibilities will include helping with everyday healthcare issues while developing an established patient relationship that allows for seamless continuity of care. This provider will also need to help educate patients about their care plans and be interested in participating in community-related educational seminars or workshops.

Minimum Qualifications:
Graduate of an accredited nurse practitioner program or completion of Master of Science Degree in Nursing/Graduate of AMA accredited physician assistant program. Licensed or ability to obtain licensure through the Wisconsin Board of Medical Practice. Ability to obtain and/or maintain DEA certification for the State of Wisconsin. Meet threshold criteria/qualifications for Credentialing and Privileges. Work schedule: .75 FTE - 60 hours per bi-weekly pay period ABOUT WESTERN WISCONSIN HEALTH
