Job Posting: Home Health Physical Therapist *Confidential application process* Position Summary: As a Home Health Physical Therapist, you will play a crucial role in helping patients regain their independence and improve their overall well-being. You will assess patients' physical conditions, develop personalized treatment plans, and provide therapeutic interventions to address various musculoskeletal, neurological, and cardiopulmonary conditions.
Responsibilities: Conduct comprehensive physical evaluations to assess patients' functional abilities and identify treatment goals. Develop and implement individualized treatment plans, including therapeutic exercises, modalities, and activities of daily living (ADLs) training. Monitor patients' progress and adjust treatment plans as needed. Educate patients and their families about self-care techniques and home exercise programs. Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated care. Maintain accurate and up-to-date patient records. Adhere to all company policies and procedures, as well as state and federal regulations.
