JOB SUMMARY
The hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting observations, and documenting care performed to ensure the quality, continuity, and safety of a patient's care.
DUTIES & RESPONSIBILITIES Perform personal care needs and promptly attend to patient requests Ensure patient safety and proper use of equipment Provide personal care, including baths, oral hygiene, and changing bed linen Assist with dressing, undressing, and meal preparation Take and record vital signs and provide proper care for the patient's skin Report patient condition and significant changes to the Case Manager Assist with ambulation, exercise, and toileting Perform simple procedures as ordered Keep patient's living area clean and orderly Adhere to documentation and care procedures Participate in quality assessment performance improvement activities JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) High school degree Valid CNA certification Ability to read and follow written instructions and document care given Understanding of hospice philosophy and comfortable providing specialized care Excellent references from previous employers Self-directing with the ability to work with little supervision Empathy for the needs of the ill, injured, frail, and impaired Current CPR certification Good oral and written communication skills Good organizational skills Licensed driver with insured automobile in good working order
