Title: Hospice Certified Home Health Aid (CHHA) - Hybrid
Department: Quality - Hospice
About The Role:
We are seeking a compassionate CHHA to provide personal care services to the terminally ill hospice patient population, provide compassion and understanding to the family in times of bereavement, and work as a solid member of the Hospice team in providing end of life care according to guidelines and care plans.
What You'll Do: Assist hospice patients in completely ADLs under the direction and ongoing supervision of the registered nurse. Complete accurate charting regarding changes in patient condition and keep overall accurate record of patient visits. Provide emotional support and compassionate care to the patient and family as a unit.Report any changes in patient condition or family situation by calling Hospice office and notifying the overseeing RN. Practice standard precautions according to OSHA regulations and policy. Adhere to all Hospice department policies and procedural guidelines. Maintain current CHHA certification as required by the state of CA. Qualifications: CHHA in CA Ability to work independently and think critically. Acceptance of and adaptability to different social, racial, and religious ideologies. Valid CA Driver's license and reliable transportation. Who We Are:

The office is at 1668 S. Garfield Avenue, Alhambra, CA.This is a full-time position, M-F 9A - 5 P. The target compensation for this position is $22 - $26 per hour. Additional Information:
