What We Are Looking For

A Medical Social Worker to join our growing team! The Social Worker will partner with the Luminary clinical team to provide psychosocial support to the patient/family unit. Your support will impact many lives, and you will find meaning in your work every day.

As a Luminary Social Worker you will:
Provide coordinated care wherever the patient calls home Provide counseling services and supportive interventions including crisis intervention as needed Assess Psychosocial needs Assist patient/family in community resource planning and access Help patient/family navigate their ongoing healthcare journey Facilitate discussions about advance care planning and health care directives and end of life planning Help families cope with grief, anticipate difficulties and resolve conflicts Work as a team to ensure patient/family have best possible end of life experience What We Offer Work/Life Balance Supportive teammates including leadership One on One patient care, more time with your patient/family Competitive pay and comprehensive benefits package: health insurance, PTO, generous 401k match Excellent mileage reimbursement Qualifications Master of Social Work degree required Active LSW license in Ohio Minimum one year experience in healthcare, hospice or palliative care preferred Valid Ohio driver's license, reliable transportation and valid automobile insurance Proficient in Microsoft office applications Effective verbal and written communication skills Excellent interpersonal skills Ability to exercise professional judgement in complex situations
