The Case Manager is the primary link between Housing Navigation Program clients and the Program and provides direct support and case management services. This position requires staff to meet at least monthly with Program clients to obtain status of their goals and documentation; to liaison between clients, the City and County of San Francisco government program staff, and other Program stakeholders; and to support clients to achieve personal and professional development goals for three (3) years, with new clients rotating in as necessary. The Case Manager also completes data entry and documentation in Awards and other reporting systems and enters data into City and County of San Francisco data entry systems to secure client housing vouchers. The Case Manager is responsible for assisting clients with meeting individualized goals while connecting them with resources in the community. The position is collaborative and coordinates care on an interdisciplinary team.
As with all UC staff, this position is responsible for adhering to the performance standards set forth in the UC Staff Employee and UC policies and procedures.

QUALIFICATIONS: Required: Bachelor's Degree in social work or another human services field; or three (3) years' experience working with system-involved youth from foster care, juvenile probation, or similar settings where clients are likely to have challenges with being responsible, have mental illness, and/or histories of substance abuse.Knowledge of Microsoft Offices applications and data base systemsDemonstrated skills in case work, group work, and case coordinationExcellent interpersonal skills (verbal and written)Demonstrated ability in facilitating and linking relevant services to address resident/client needsAbility to maintain a positive perspective in the treatment and care of young adults with mental, emotional and social issuesOther important competencies include the ability to motivate and maintain effective working relationships with staff and partners and excellent time management skillsPreferred: Master's degree in a social work or related human service fieldA minimum of two (2) years of experience working with system-involved youth from foster care, juvenile probation, or similar settings where clients are likely to have challenges with being responsible, have mental illness, and/or histories of substance abuse.Experience working in a scattered-site housing program with case management servicesRESPONSIBILITIES INCLUDE (but not limited to): Build relationship with clients referred to the program; ensures clients complete and sign all required documentation; and work with the San Francisco Housing Authority to acquire client housing vouchers.Manage a caseload of up to 50 scattered-site young adult clients. Develop a relationship and maintain individual contact with each participant at least monthly to discuss goals and track progress towards goals and problem solve. Each contact is documented in Awards within 3 days of contact.Maintain ongoing contact with clients' support system such as: natural supports, family, referring agency, and community resources.Participate in relevant program and City team meetings. Serve as a representative of UCG and liaison with these parties.Support clients to develop case plans, including individualized goals. Maintain current documentation in Awards and other program data entry systems.Track and monitor client's achievement of goals to meet contractual requirements. Document status in Awards monthly.Attend required trainings, and meetings as directed.Organization Duties:Timely and accurate submission of timecards to supervisorTime away from the Office, Program and/or Home (i.e. vacations, personal time off, leaves of absence) must be pre-approved by supervisor and requested based on process outlined in the UC Employee Handbook.Maintain knowledge of staff handbook, policies, procedures, manuals, and pertinent documentationOther duties as assigned.ESSENTIAL DUTIES and QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
