Immediate Hiring For Physical Therapist - $5,000 Sign-On Bonus

An established nonprofit in the San Francisco Bay Area committed to providing affordable, comprehensive, compassionate, and quality home care services is seeking a qualified Physical Therapist with experience working in an outpatient setting. The ability to communicate in Cantonese and Mandarin is preferred but not required.

Responsibilities and Duties for Physical Therapist (PT):
Physical Therapist will perform an assessment & evaluation in the home and develop a total care treatment plan for each patient.Help patients with mobility, movement, and help in regaining their independence.Conduct chart reviewing for other Physical Therapist's notes and assure proper documentation are in compliance with state, federal, and CHAP standards. Communicate and coordinate with patients' physicians to ensure a quality level of care. Evaluate and report patients' progress. Be part of the QAPI team in strategic planning and identifying opportunities for additional or improved services to address unmet patient care needs.

You must have:
