The Intensive Case Manager (ICM) performs intensive case management activities for members who are seriously mentally ill (SMI) and identified as having potential high risks and/or needs. The ICM works as a member of a clinical team to assess member needs, decrease member hospital and emergency room utilization, assist with physical health and managing chronic health conditions that impact members' overall well-being, increase member service and referral utilization within the community, reduce homelessness, and increase member self-sufficiency and independence. The ICM acts as a member advocate, coordinates services and discharge plans, and manages and assists in crisis interventions.

MINIMUM QUALIFICATIONS: Bachelor's degree in behavioral health related field; or Bachelor's degree in non-behavioral health related field and at least one (1) year of full-time direct care experience in behavioral health; or Associate degree in behavioral health related field and two (2) years of full-time direct care experience in behavioral health experience; or High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health. Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire. Must have reliable transportation, valid AZ Driver's License with state minimum insurance coverage and ability to meet and maintain Copa Health's acceptable driving requirements. TB Test clearance required at the time of hire and annually. Bilingual (English/Spanish) preferred JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO) Maintains a reduced caseload of SMI and high risk members, including frequent in person and community contact with members, to address physical and behavioral health needs, along with coordination and connection to care. Coordinates the complete functional risk assessment of members on their caseloads by arranging for clinical evaluations, psychiatric consultations, medical consultation with PCP and identified specialists, psychological testing, functional assessments, rehabilitative and social needs. Assists in monitoring physical health and value-based outcomes for their assigned caseload through teaching and interpreting to members the importance of physical health and wellbeing, including scheduling and attendance of regular primary care and specialist appointments, discussing prescribed medications including monitoring usage and refilling of member prescriptions, etc. with the guidance and oversight of the team nurse. Provide support to members and families in accessing additional needed services and obtaining entitlements, including identification of funding services. Works under the supervision of a licensed individual (e.g. team Nurse, Clinical Director, Program Director), as well as with the Prescribing Clinician, and other members of the clinical team, to ensure the timely development and implementation of the individual's service plan (including all necessary assessments) and their crisis plan. Monitors and follows-up the implementation of the written service plan via home visits, linking members with necessary providers, assisting in gaining access to needed medical, psychiatric, social, educational, and other services, while maintaining frequent contact throughout the course of treatment. Coordinates with all parties involved in member's treatment according to policy and procedure and clinical protocols. Presents available treatment services for use by the individuals and/or family members, reviews ongoing treatment needs and services, and assists with the stabilization of the individual and/or family after a crisis. Responsible for arranging and/or providing transportation for members to necessary services including physical and behavioral health appointments, as outlined in their service plan and as needed. Maintain current and complete records and reports, and completes all clinical documentation in a timely manner, in accordance with Copa Health and regulatory standards. Meets member engagement goals defined by the department and Copa Health. Participates in daily team meetings as well as any additional individual treatment teams or other team communication meetings as required. Adherence to all agency policies, licensure, and training requirements, including successful completion of the Observation of Self-Administered Medications (OSAM) training, and ensures compliance & adequate services. Additional job duties may be required to meet the needs of the program and/or department.
