The LCM will consult with members to determinetheir needs, develop individualized action plans, and work with care teams to manage themember experience effectively. Providing emotional support, resolving administrative issues,and ensuring timely access to care are key aspects of the position. Compassion, healthcareknowledge, and exceptional customer service are essential qualities for assisting members inbecoming self-sufficient in health.Responsibilities:•Interview members to assess medical and social needs and provide education abouttheir condition and medication, while developing individualized care plans.•Respond to member inquiries and concerns, ensuring adherence to hospital and legalrequirements.•Collaborate with interdisciplinary teams, locate medical and social resources, andcoordinate social service plans.•Advocate for members, consult with healthcare providers, arrange appointmentsand treatment plans, evaluate member progress, and assist with healthcarebarriers.•Maintain empathy and professionalism while contacting members and families.•Supporting behavioral health coordination, Substance Abuse and CommunityResources.•Additional duties as assigned.Requirements/Qualifications:•Must have a valid California driver's license and valid vehicle insurance.•High school, diploma or GED. (Required)•Minimum of 1 year experience in case management, member care, customerservice, call center, or member care. (Required)•MA certificate or Medical Terminology knowledge. (Preferred)•Excellent communication, interpersonal, customer service and organizational skills.•Computer skills for documentation, email and chat support.•Proficient skills in working independently and collaboratively in a team to providemember care.•Proficiency in multitasking, organization, and attention to detail is required foreffectively providing care to multiple members simultaneously.•Candidates should exhibit the capability to utilize resources effectively forproblem-solving while maintaining composure under pressure in a compassionatemanner.•Applicants must demonstrate proficient speaking, reading, and typing abilities andpossess strong proofreading skills.Working Environment / Physical Requirements:•Must be able to travel using personal vehicle to complete outreach visits.
