Lead Care Manager LCSW Job DescriptionThe Lead Care Manager LCSW is responsible for providing leadership and oversight to the care management team. The ideal candidate will have a Master's Degree in Social Work from an accredited program, California Clinical Social Worker License, 2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute), 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers, and 2-5 years of work experience.The Lead Care Manager LCSW will be responsible for the following duties:Developing and implementing care management plans for patientsCoordinating care with other healthcare providersMonitoring patient progress and outcomesProviding education and support to patients and their familiesManaging the care management teamAdministering the care management programReporting on program outcomesThe Lead Care Manager LCSW must be a highly motivated and organized individual with excellent communication and interpersonal skills. The ideal candidate will be able to work independently and as part of a team, and will have a passion for providing high-quality care to patients.
