A healthcare network in New York City is is currently seeking a Licensed Master Social Worker (LMSW) and/or Licensed Mental Health Counselor (LMHC) to join their growing team in Brooklyn.

About the Opportunity: Schedule: Monday to Friday Hours: 8:30am to 5pm (2 late days a week 10:30 to 7pm) Patient Population: Ages 55+ (asylum seekers, undocumented, etc.) Caseload: 55 clients Responsibilities:

The Social Worker (LMSW / LMHC) will: Complete admission screening and conduct psychosocial assessments Make treatment recommendations and participate in treatment planning Provide social casework services; provide individual, group, and family therapy Develop and implement discharge plans and and coordinate services with community providers Review the individuals psychiatric, medical and other case records and discuss issues with families and other members of the treatment team Establish and maintain a therapeutic relationship with individuals and their families Qualifications: Bachelor's Degree Valid NYS Licensed Master Social Worker (LMSW) and/or Licensed Mental Health Counselor (LMHC) Your privacy is important to us.
