Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status. Functions in accordance with the professional standards of the Ohio board of nursing. Completes/submits required documentation. Delivers effective relationship based nursing care. Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes. Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations. Develops self and contributes to the professional practice of nursing. Displays leadership that influences optimal clinical and operational outcomes. Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. 100% travel to the patient's homes to provide care. This position is in Home Care Services. Monday-Friday with rotating weekends and holidays. This LPN will support Elyria and surrounding areas. Qualifications: Other Accredited Program Completion of accredited practical nurse program or currently enrolled in a nursing program required. 2+ years Nursing Experience (Level II only) required. Knowledge of LPN scope of practice according to the Ohio board of nursing. Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices). Level II Only: 18 CNE's per year. • 3 CNE's in clinical specialty • At least 5 CNE's in Leadership • Other CNE requirements will be agreed upon with manager. Level II Only: • 3 Peer Reviews. • Committee Involvement. • 5 criteria with evidence of each. Knowledge of LPN scope of practice according to the Ohio board of nursing required. Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices). Licensed Practical Nurse (LPN), Ohio and/or Multi State Compact License Upon Hire required. Basic Life Support (BLS) Upon Hire required.
