Functions in accordance with the professional standards of the Ohio board of nursing. Completes/submits required documentation. Delivers effective relationship based nursing care. Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes. Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations. Develops self and contributes to the professional practice of nursing. Displays leadership that influences optimal clinical and operational outcomes. Performs other duties as assigned. Complies with all policies and standards. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. 100% travel to the patients homes to provide care. Part time 24 hours per week. The candidate will cover the Western Market of Cuyahoga County. Monday-Friday with rotating weekends and holidays. This position is in Home Care Services. 100% travel to the home of patients who will receive care. The territory will be:

Qualifications: Other Accredited Program Completion of accredited practical nurse program or currently enrolled in a nursing program required. 2+ years Nursing Experience (Level II only). Knowledge of LPN scope of practice according to the Ohio board of nursing. Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices). Licensed Practical Nurse (LPN) in the State of Ohio required. Basic Life Support (BLS). IV certification (unit depending). Valid ODL/car insurance
