As aLicensed Practical Nurse (LPN) Visit Nurseyou will:
Providein-homedirect patient care according to theprovidersprescribed plan of care (and nursingscope of practice) while closelymonitoringprogress/recovery Complete skilled assessments and provideexceptional patient-centered care in the home Ensure an accurateevaluation and implement a comprehensive care plan that addresses patient needs Communicate patient conditions and collaborate with appropriate providersto deliver care when patient needs evolve Complete all clinical documentation in accordance withagency protocol and Medicare/Federal guidelines Provideeducation to patients and family members involved inpatientscare. Work to decrease readmissions by promoting preventative care and ensuring continuity of care To qualifyfor aLicensed Practical Nurse (LPN)positionwith us, you will need:
Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as anLPN/LVN in the state(s) in which he/she practicesand have atleast1200 hoursof experience as an LPN/LVN or completion of the Office's preceptor program Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN)in the state associated with this position Current CPR/AED/BLS/First Aid certification Reliable transportation to/from care sites and/or work locations Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting At Interim HealthCare, we know that being our best is non-negotiable thatswhy we treatyourfamily like our own.
