Responsibilities: Provide one-on-one care for patients in their homes Provides skilled nursing services as per care plan. Documents patient observations, interventions, and goals. Delivers and documents patient care during visits. Communicates changes in patient condition to the clinical team. Meets productivity standards and clinical competencies. Follows standards for technology and documentation. Qualifications: Active LVN state license Must have home health / hospice experience Knows how to use HospiceMD for charting Current CPR/BLS Certification Recent physical and TB test (within one year) Driver's License or State ID Reliable transportation and insurance ** THE RATE IS PER VISIT **
