Medical Records

• Plan, develop, organize, implement, evaluate, and directthe Medical Records Department and its programs in accordance with departmentalpolicies, procedures, and standards.
• Review department policies, procedures, manuals,position descriptions, etc., at least annually and participate in makingrecommended changes.
• Audit and complete on-going review of all patients'clinical records to ensure documentation and performance compliance.
• Maintain current, overflow, and discharged record filingsystems. Completes medical record thinning and filing on a regularbasis. Files active and inactive records in accordance with establishedpolicies.
• Assemblesand analyzes discharge records; obtains discharge summary for each dischargerecord, follows up as indicated.
• Ability to use ICD-10-CM coding.
• Ability to use CPT/HCPCS coding systems.
• Prepares and sends record requests for Medicare appealsto the Quality Improvement Organization.
• Coordinatesand prepares medical records for new admissions.
• Coordinatesoutside specialty/follow-up medical appointments along with transportation andescort (if appropriate) to and from the appointment if required.
• Reviewsphysician progress notes for additional diagnoses, indexes, and codesdiagnosis.
• Credentialsnew physicians and providers and updates their files as needed.
