Description: Medical Social Worker - Discharge Planning - BMH - Days

The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to:
Identifies patients with barriers to a safe discharge plan. Completes a psychosocial assessments on all patients. Documents all clinical assessments, notes, and referrals in Epic. Completes screens and coordinates all new skilled nursing facility and rehab facility referrals. Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault. Educates patient/family about post-acute care options and the role of choice. Educates patient/family about managed care and payor issues Full time, 9-5 pm, M-F and one weekend per month. This is a 1199 position.
Preferred Criteria At least one year of inpatient Medical/Surgical experience Discharge planning experience Experience with Epic and Allscripts Requirements: Master's Degree in Social Work (MSW) New York State License as a Licensed Masters Social Worker (LMSW) Join a healthcare system where employee engagement is at an all-time high.
