As a MLTC-Intake RN, you will be responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family. Ideal candidate will be eligible for a $20,000 sign on bonus
Responsibilities:
The role of the Intake RN at RiverSpring Health Plans will be responsible for knowing require outreach activities within the community and the assessment of eligibility and safety of potential members. Collaborate with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members. Communicate with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.
Qualifications:
Must be a self-assured professional with a current New York State Registered Nurse License required. Bachelors degree in Nursing preferred. One (1) year experience in an acute care setting required. Must have UAS- NY experience. Home care and management experience preferred. MS Office required. Effective oral, written, communication and interpersonal skills required. Bilingual Spanish Required Compensation: $95,000 - $105,000 Annually
