Msw Social Worker - Emergency Department

Responsibilities: Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems. Duties also include, but are not limited to, conducting psychosocial assessments, making a psychosocial diagnosis, planning and implementing effective treatments, evaluation, education, supervision, coaching and mentoring of the patient (client) and family members. The position consults with, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources in the overall care of the patient. This is a medical social work department we are paired alongside nurse case managers who support with discharge planning. We assess social determinants of health by completing biopsychosocial assessments with the goal of increasing access to quality health care and reducing barriers to discharge. We provide patients and families with emotional and resource support by providing interventions that are strength-based, focused on the whole-person, trauma informed and span across the life cycle of our patients We collaborate with the multi-disciplinary teams on how the illness or injury may impact the patient's, family, employment and living situation. We promote health and reduce health inequities by advocating for, educating and empowering patients and families.
