$24 per hour
The Case Manager will complete contractually required enrollment steps, a comprehensive assessment and corresponding service planning. This work will be done in collaboration with each youth participant to elevate their experiences, their strengths, their interests, and their voice. While in the program, each participant will meet with the Case Manager regularly to work on barriers to program persistence and success, including the provision and follow through with referrals to other service providers. The Case Manager will also complete all necessary data entry related to this work and support the larger team with other program activities, such as outreach and recruitment, workshops, post-exit placements, etc.
Essential Duties and Responsibilities:
Maintains full caseload, complete with ongoing individual service plans
Assess ongoing changes in behavior, circumstances, or conditions that may affect client safety
Engages /involves client and sponsor in the casework process
Prepares, compiles, and submits accurate case files
Serves as a liaison with stakeholders (i.e.. legal providers, immigration court)
Actively participates in seminars, workshops, educational programs
Completes home visits to meet with sponsor (if applicable)
Maintains contact with clients through phone calls and home visits (if applicable)
Consults with supervisors to determine decisions as they relate to the safety, well-being and permanency of patient
Demonstrates client progress toward risk reduction, achievement of treatment goals and positive case outcomes
Maintains accurate records, files, forms, statistics and additional relevant information in accord with agency policy, licensing, and/or funding requirements
Conducts in-home assessments (if applicable)
Performs other duties as assigned/necessary
Minimum Requirements:
Bachelor's degree required. A master's degree or additional, substantial professional development in a related field preferred.
A minimum of 3 years' experience in a case manager or comparable role.
