*Nurse Practitioner - Homebound Elderly Care*

*HCRI *is seeking a compassionate and dedicated *Nurse Practitioner (NP)* for a non-profit organization providing support to individuals and families facing the devastating effects of medical crises. Providing in-home medical care to elderly patients in Brooklyn, NY.

*Nurse Practitioner Key Responsibilities:*
* NP will conduct in-home visits for scheduled appointments and sick visits
* NP will work M – F plus weekends, flexible scheduling options
* NP will offer personalized care to approximately 15 – 20 patients daily
* NP will provide support to field RNs as needed for escalation cases
* NP will order and interpret labs and prescribe medications as needed
* NP will have support of an MD if needed
* NP will monitor and manage patients appropriately
* NP will document all patient care issues in permanent medical records

*Nurse Practitioner Qualifications:*
* Current applicable state NP license
* Current DEA license not required can be obtained through employment
* BLS Certification
* Strong background in hospital settings, with critical care experience
* Compassionate and warm personality, especially in delicate situations with seniors
* Experience with Medicare /Medicaid billing
* Flexible for on-call/virtual after hours

*Nurse Practitioner Benefits*:

· Salary Range: $125. - $150. Per hour DOE

· Sign on Bonus: $2500

· Mileage reimbursement for travel
