Description
The Department of Family, Population & Preventive Medicine is seeking a Nurse Practitioner or Physician Assistant to provide direct care to patients in the Division of Family and Community Medicine at Stony Brook, in collaboration with an Attending Physician.
Responsibilities:
Participate as part of a multidisciplinary team to provide continuity of care and follow-up.
Provide direct care, counseling, and teaching to patients and families according to established collaborative protocols and standards of care.
Evaluate outcomes of patient care and makes necessary revisions in care plan.
Through patient education and counseling, incorporate the concepts of health maintenance, prevention, and promotion into daily practice.
Identify own learning needs and assume responsibility for maintaining current knowledge and competency in clinical practice area.
Participate in the quality assessment/improvement program for the service.
Qualifications
REQUIRED QUALIFICATIONS:
Nurse Practitioner:
Current NYS RN Licensure. NYS certification as an FNP. Completion of an accredited Nurse Practitioner course of training. Masters of Science degree (foreign equivalent or higher), licensed in the State of New York and board certified by a Nurse Practitioner Certifying body (AANC, AANP, AACN, NCC, PNCB). Excellent communication skills (both oral & written), interpersonal and organizational skills are required. Basic Computer skills required. A minimum of one year experience working in a outpatient primary care setting.
Physician Assistant:
Bachelors Degree (foreign equivalent or higher) from an accredited Physician assistant (PA) school, current license and certificate to practice as a Physician Assistant (PA-C). Excellent communication skills (both oral & written), interpersonal and organizational skills are required. Basic Computer skills required. A minimum of one year experience working in a primary care outpatient setting.
PREFERRED QUALIFICATIONS:
Three or more years experience working in an outpatient ambulatory care setting. Experience working in patient centered medical home.
