Nursing Home Patient Access Liaison

Western Wisconsin Health is seeking a full-time Nursing Home Patient Access Liaison (PAL). The Nursing Home PAL is responsible for improving the partnership and overall patient care experience for external nursing homes, assisted living homes, and group home partners. The Nursing Home PAL enhances the work life of primary care clinicians by taking on tasks that can be completed by nonclinical staff. They are the first point of contact for non-clinical needs and exemplifies personal warmth professionalism and is discrete, while offering solutions in anticipation of needs. Minimum Qualifications High school diploma or GED Associate degree or 2+ years of experience in a related field Knowledge, Skills and Abilities Medical terminology Ability to answer and manage a large volume of phone calls Exhibit excellent communication and organizational skills Ability to manage conflicting priorities and handle multiple tasks/projects concurrently Working knowledge of Microsoft Office (Excel, Word, PowerPoint, Outlook) and electronic medical records Maintain the integrity of highly confidential business information Display and promote high standards of ethical conduct and behaviors consistent with organizational standards Work schedule: 0.8 FTE - 64 hours per bi-weekly pay period Monday through Friday ABOUT WESTERN WISCONSIN HEALTH
