Description
The MDS/Enrollment Nurse is responsible for ensuring potential and or current PACE participants are meeting the clinical requirements according to the Minimum Data Set clinical (MDS) assessment. The individual in this role conducts home visits and assists in completing the intake Prospect sheet for visits. The MDS Nurse is responsible for managing all aspects of the clinical assessment. the MDS/Enrollment Nurse collaborates with other departments to provide clinical information.
Duties & Responsibilities:
Conducts home visits to provide clinical assessment for prospective participants.Performs all necessary actions to complete initial and annual state required clinical assessments. Provides clinical information to the IDT and contributes to the completion of the initial MDS and provisional care plan, acts as a liaison between PACE and the pre-enrollees current home services.Submits completed MDS to EOEA/OLTC.Completes reassessment and submission of expired clinical assessments.Assures presence of completed and signed Health Care Proxy, Medical Record Form and other designated forms. Provides detailed clinical information of potential applicants to IDTs to assist them in preparing for transition of new enrollees.

4. Attends PACE and other required trainings annually. Qualifications Minimum Basic Knowledge: Home Care experience preferred.Experience with ICD-10 coding.Prepare concise and clear visit assessments.Excellent organizational skills.Good knowledge of Microsoft Word applications including Excel, Access and Power Point.Second language is strongly preferred (English/Spanish, English/Portuguese Creole, English/ French Creole).Knowledge of electronic health data systems including PACE CARE Online (PCO) is preferred. Experience & Qualifications: Current MA Registered Nurse (RN) License and BLS required.One year of experience working with an elder population, 2 + years experience in a healthcare setting.Excellent verbal and written communication skills.Ability to identify ADL and Skilled Nursing needs.Ability to work independently and as a member of a team. Certifications/Licenses:
MA valid drivers license with appropriate automobile insurance required.
