Overview:
Performs activities of a supportive nature, involving direct and indirect care delivery.
Responsibilities:
Provides direct care delivery for direct observations of one-to-one patients for maintaining safety standards as per policy. Documents /records on the observation flow sheet every 15 minutes the behavior and the communication (verbal or non-verbal) of the patient. Maintains constant observation of 1:1 patient care at all times including securing of relief for incoming nursing personnel (at the change of shift), and relief for breaks and meals. Receives endorsement/report from off going assigned PC about the current status of the patient. Maintains patient safety standards at all times. Reads and signs a copy of the 1:1 Protocol Staff Assignment Sheet. Assists patients and participates in group activities that promote patients social, leisure time skills and activities of daily living (ADL). Observes the activities of patients and reports behaviors to the Registered Nurse (RN/AHN). Must report all evidence of contraband and any change in patients behavior to the Assistant Head Nurse AHN/Charge Nurse on duty. Assists with the admission, transfer and discharge of patient as per IMC policy. Obtains vital signs, (temp, pulse, resp., blood pressure), height and weight and records results on Vital Signs Sheet as per IMC Policy. Observes and assists patient(s) with meals. Reports to AHN/Charge Nurse any intake/output or dietary problems. Ensures and demonstrates safe use of equipment such as Wheelchairs, Geri-chairs, etc. Identifies problems and obstacles affecting the delivery of services to patients and reports same to the charge RN or Nurse Manager. Escorts patients and accompanies patient to appointments in-house/outside hospital premises. Obtains supplies and equipment as needed to assist in the delivery of quality patient care. Assists patients in providing Activities of Daily Living (ADL). Assists in maintaining cleanliness of patients area (e.g., bed making, remove foods from patients room, doing laundry). Assists in completing 15 minutes (or as per MD orders) surveillance sheet. Talks, listens and relates to patients in a positive manner. Maintains confidentiality and security of patient information in accordance with IMCs Policy. Assist in orientation of peers. Attends and participates in nursing staff meetings. Practices and adheres to IMCs Infection Control Policies. Participates in Performance Improvement activities that enhance the quality of patient care. All other duties as assigned.
Qualifications:
Education: High school diploma or GED or equivalent experience required. Knowledge and Skills: Excellent verbal, written, communication skills. Basic computer knowledge. EKG. Phlebotomy. Licenses, Certifications: Medical Assistant or Patient Care Technician certification with EKG and phlebotomy skills, required. Physical Requirements: Involves walking, sitting, talking, listening, lifting and bending. May include some repetitive motions.
