Patient Relations Representative

An established Home Care organization is looking for a Patient Relations Representative for their Brooklyn location. The organization is a fully licensed and bonded home health care agency serving Brooklyn, Manhattan, Queens, The Bronx, and Staten Island, as well as a Fiscal Intermediary in most New York counties, and is the home care agency of choice for thousands of families. This role involves acting as a liaison between patients, their families, and healthcare providers to address concerns, enhance communication, and improve overall patient satisfaction.Responsibilities:Patient Advocacy: Serve as the primary point of contact for patients and their families, addressing concerns, complaints, and requests with empathy and professionalism.Communication: Facilitate effective communication between patients, families, and homecare providers to ensure understanding of care plans, procedures, and policies.Record Keeping: Maintain accurate and detailed records of patient interactions, complaints, and resolutions in accordance with company policies and confidentiality standards.Quality Improvement: Participate in quality improvement initiatives and contribute to the development of policies and procedures aimed at enhancing patient satisfaction and care in the homecare environment.Support Services Coordination: Assist patients and families in accessing additional services such as financial counseling, social work, and support groups.Qualifications:Minimum of 2 years of administrative experience, preferably in a healthcare or homecare setting.Knowledge of the NHTD program and related healthcare services is a plus.Commitment to the mission and values of the NHTD program.Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).Excellent written and verbal communication skills. Ability to handle sensitive information with discretion.
