Currently serving the following cities inWayne County: Dearborn, Detroit, Garden City, Westland, & Lincoln Park (subject to change as recipient needs change).Activities (most critical to least critical)Performs physical therapy evaluations, develops and implements treatment plans focusing on improving/restoring independence and physical function. Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests.Prepares client assessment and periodic evaluations and summaries, develops a plan of care and writes clinical/progress notes after weekly visit and incorporates information into the client record.Implements client treatment to relieve pain and/or restore function to maintain maximum performance.Maintains clinical documentation and progress notes. Submits all required paperwork in a timely manner according to ExpertCare policy. Periodically reports progress of client's treatment to the physician and assists physician in determining any necessary changes in the treatment program. Instructs clients and their families in the use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, etc.).Instructs the client and pertinent family members with all aspects of the client's total physical therapy program.Attends therapy staff meetings and in-service presentations. Participates in staff and agency development activities.Participates in annual Person-Centered Planning meetings and Periodic Status Review meetings to provide insight and update on treatment planning and implementation.Hard Skills and EducationCurrent state PT licenseCurrent certification with APTA Valid state driver's licenseExperience2+ years' experience working as a Physical Therapist in a health care setting.1+ year experience in a supervisory roleMinimum 6 months experience utilizing an EMR for documentation.** $1,000 Sign-On Bonus potential**Working with a minimum of 10 recipients**Additional compensation for evening and weekend visits**Benefits:Flexible ScheduleWeekly PayVirtual Interview ProcessBenefit Opportunities Sign On Bonus potential.At ExpertCare, we strive to make a difference and impact in the lives of our recipients and employees!
