Responsible for evaluating individuals with mobility dysfunction and provide a plan of care that is specific to the individual's goals related to functional independence. Corporate SettingThis position is also responsible for providing support to the Rehabilitation Manager in the design, development, implementation, and monitoring of rehabilitation programs to improve outcomes, processes, and participant experience. This position is also involved in the onboarding and training of new rehabilitation staff members and will provide coverage at the PACE sites as needed.Minimum Requirements1. Physical Therapist license issued by the State of California Physical Therapy Board required.2. Prior experience working with the frail elderly (Geriatric) and disabled populations preferred.3. Current BLS certification also required.4.
