ARIZONA GENERAL HOSPITAL Responsibilities The Therapist provides patient care, including evaluating the patient for treatment, providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management, while delivering quality patient services. Qualifications Minimum: Internship or rotation in appropriate clinical setting required. Graduate of an accredited Physical Therapy Program required. Licensed by AZ State Board of Physical Therapy as a Physical Therapist. American Heart Association BLS. Bachelor's degree. Preferred: Prior use of electronic health record preferred. Master's or Doctorate degree in appropriate clinical area preferred. 2 years progressive experience in clinical area performing patient clinical care preferred.
