GENERAL SUMMARY:
Physician Assistants (PA) and Nurse Practitioners (NP) are graduates of an accredited educational program and are nationally certified and state licensed professionals. The scope of practice is determined by state law and licensure, facility policy, education, professional judgement, personal experience and practice agreement. Physician assistants are independent prescribers of Schedule II-V controlled substances; using their own DEA number and signature.
Rapid Response Team Hospital-wide 24/7 coverage of all RRT calls, including ancillary areas. Main responsibility is the efficient assessment and stabilization of patients by ordering and interpreting variety of diagnostic tests, performing therapeutic interventions, and transferring patient to appropriate level of care. All of this done in collaboration with attending and/or consulting physicians, respiratory therapists, nurses, and ICU residents when necessary.
Pulmonary Embolism Response Team Acute risk stratification for all PE patients in ER or inpatient units for potential interventions. Responsibilities include performing acute assessment of patient then initiating conference call with PERT attending physician, cardiologist, interventional radiologist, and others to determine appropriate therapeutic intervention.
Acute Stroke Team Responding to all code strokes in ER and inpatient units. Responsibilities include evaluating patient and performing time sensitive diagnostic and therapeutic interventions in collaboration with on-call neurologist, ER physicians, and neuroradiology interventionists.
Transition of Care Notes Assisting with admitting patients from the ER to the floor by performing transition of care notes, reconciliation of medications, and ordering appropriate diagnostic and therapeutic tests/interventions.
Management of ER Hold Patients Management of critical lab results, decompensating patients, and responding to all nursing concerns/requests
ESSENTIAL DUTIES:
1. Utilization of a high degree of discriminative judgement while providing medical management including assessment of medical and psychosocial status, comprehensive history and physical examination and condition assessment. Order and interpretation of appropriate laboratory tests, diagnostic studies and therapeutic modalities with the development of diagnostic impressions and a comprehensive plan of care. Plan of care will be based on current guidelines and with the participating physician when necessary.
2. Request consultation from health professionals as indicated by patient condition.
3. Write orders for medications within prescriptive authority pursuant to state and federal regulations and the practice agreement.
4. Perform procedure as indicated by patient need, PA education, and experience and practice agreement.
5. Document all medical findings in the form of H&P/Transition of Care, progress notes, procedure notes, discharge instructions, and discharge summary.
6. Instruct, educate and counsel patients and families, as appropriate, on health status, test results, disease process, and plan of care, preventive care and health promotion.
STANDARD QUALIFICATIONS:
A. Education/Training:
+ Graduate of AMA recognized PA program that is accredited by the Accreditation Review Commission on Education for Physician Assistants and meet requirements of the state or Masters in Nursing.
B. Work Experience:
+ Minimum of 1 year of previous APP experience strongly preferred.
C. Certification/Licensure:
+ Certified by the National Commission on Certification of Physician Assistants
+ Licensure as a Physician Assistant or Nurse Practitioner by the state of Michigan (Acute Care NP is preferred.)
+ BLS and ACLS is required
+ NIHSS is preferred
+ ENLS is preferred
D. Other Qualifications:
+ Demonstrates critical thinking, possesses excellent oral and written communication skills, team oriented and collaborative outlook with a focus on patient and family centered care.
