Job Description Responsible for the provision of nutritional care to patients/residents from different populations and disease states including nutrition education to patients/residents and/or family members in Long Term Acute Care Hospital and Skilled Nursing Facility. Participates in performance improvement activities. Acts as a member of the interdisciplinary team.
Duties & Responsibilities • Timely completion of initial and periodic assessments of nutritional needs of patients/residents
• Development, implementation, and revisions of nutritional care plans
• Formulation of discharge plans if warranted for continuity of nutritional care
• Follow-up progress of patient/resident nutritional status and revision of care plans, as needed (ex: significant weight changes, pressure injuries, enteral nutrition, hospitalizations, BMI under 21, malnutrition, calorie counts, changes in condition that requires specific nutrition interventions, etc.)
• Timely completion of MDS for residents in the units assigned (per procedure and per schedule)
• Interpretation/evaluation and implementation of diet orders including enteral nutrition. Ensuring patients/resident cultural and religious requests are addressed. Forwarding requests to the Diet Office. Educating dietary staff on requests if need to ensure compliance.
• Participates in comprehensive care plan meetings for covered units and non-covered units if needed
• Provides nutrition education to patients/residents and family members
• Timely submission of daily/weekly/monthly reports of productivity
• Participates in staff meetings, in-services, and performance improvement (PI) meetings as needed
• Completes daily meal rounds to ensure conformance to prescribed diets, adequate food intake, and overall meal tolerance
• Provides in-service to Dietary/Nursing staff regarding diets/supplements as assigned by Clinical Nutrition Manager
• Communicates with Physicians, Nurses, and members of other disciplines concerning the provisions of appropriate nutritional care. Acts as a consultant on nutrition issues for other members of the interdisciplinary team.
• Visits patients to obtain a detailed nutrition history summary and determine their nutritional needs. Once needs are determined, comprehensive assessments will be completed, including recommendations that will be communicated to physicians and/or nursing staff
• Performance of daily necessary clinical functions while providing coverage for other dietitians
• Assists Clinical Nutrition Manager with menu planning and nutrient analysis as needed
• Precepts dietetic interns as needed
• Coverage in the absence of Assistant Clinical Nutrition Manager/as needed.
• Collects data and participate in department QA and PI projects
• Performs other related duties as assigned by Clinical Nutrition Manager
Minimum Qualifications Qualification Requirements:
For appointment to Assignment Levels III and IV
1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.
Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:
Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
