Title:Registered Nurse (Full-time or On-call) Department: Adult Day Services
FLSA Status: Non-exempt
Reports To: Director
Summary: Provides skilled nursing assessment, planning and care in order to maximize the comfort and health of Adult Day Services participants.
Essential Functions:
Conducts initial, quarterly, and 6-month nursing assessments of participants; develops and implements nursing care plans.
Acts as a liaison to participants primary care physicians.
Provides routine counseling to participants and caregivers; develops and implements self-medication and continence training programs and assists families in the management of incontinence.
Provides health education presentations to the participants.
Evaluates participants need(s) for evaluation and/or services by registered dietitian, speech therapist, psychiatric consultant, podiatrist, and/or other medical services; makes referrals according to the programs procedures.
Maintains appropriate nursing records in participants charts.
Oversees the administration of personal care services by program aides.
Provides in-service training for staff on health-related topics and medical consultation to other staff.
Maintains an inventory of medical supplies and equipment, medications ordered by personal physicians, and PRN medications.
Dispenses medications and disposes medications, needles, syringes, and hazardous waste materials according to the programs procedures.
Participates in morning huddles, multidisciplinary team meetings and staff meetings as schedule permits, and other meetings as assigned.
Performs other duties as assigned.
Qualifications:
Current California Licensed Registered Nurse.
Previous nursing experience working with the elderly or the chronically ill, or experience in home health, public health, or acute care preferred.
Good interpersonal skills with the ability to work well in a multidisciplinary team.
Familiar with different patient transferring techniques. Must pass a strength test for physical fitness for the job prior to employment and every 2 years thereafter: Ability to transfer participants of up to 120 lbs, stand, walk, bend/twist/turn the neck/waist, reach, grip/grasp, pinch/squeeze, exercise fine manipulation of the hands and wrist movements, kneel, squat, and push/pull up to 75% of time.
Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.
Must pass health screening for physical examination.
Must acquire CPR and First Aid certificates within 6 months of employment and maintain.
