Responsibilities:
Here's how you'll move healthcare forward in this role:
Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety. Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care Provides education and strategies for patients and families Partners with other VNA clinicians and external providers to execute best patient outcomes Documents all changes in patient's condition and updates care plans and outcomes

Qualifications:
The ideal qualifications for the Registered Nurse, Homecare, include:
Active Registered Nurse license. BSN a plus. 1+ year of supervised clinical experience as a registered nurse, preferably in home health or medsurg Excellent verbal and written communication skills. Solid interpersonal and time management skills Strong computer skills Strong problem solving skills Car and valid driver's license Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
